AC Milan travel Docs have to be in Monday

Apparently Spurs sent Milan Travel info documents out on Tuesday.
I haven't received mine yet but friends who have say they have to be returned to Club by Monday.
I know of others who also have not received their forms yet.
Luckily I still have the email (from Club) with the form (as a attachment) that they sent for Inter game.
Anyone who hasn't received theirs yet should email the ticket office and ask them to email one to them.

If you're travelling with Thomas Cook then you don't need to complete form.
If you're travelling independently then I suggest you phone the Club.
You can Fax form to them.
